Advantage Health Solutions is a locally owned company serving several counties in Indiana.  They offer group insurance as well as Medicare insurance.  They offer four Medicare Advantage Plans that are PPO network based that cover both Medical and prescription drugs.  The four plans are Advantage Choice, Advantage Select, Advantage Elite, and Advantage Enhanced.  These plans are available in Indiane to residence of the following counties:  Allen, Boon, Delaware, Hamilton, Hancock, Hendricks, Howard, Johnson, Madison, Marion, Morgan, Shelby, and St. Josephs.  Four plans is a large offering for one area, so let’s look at each one in more detail in order to see which one makes the most sense for you!

  • $200/day hospital co-pay days 1-5
  • $15 co-pay for primary and $35 co-pay for specialist
  • $175 co-pay for outpatient surgery
  • $0 co-pay for labs and diagnostic procedures and tests
  • Also offers dental and vision benefits as well as a gym membership

Advantage Select

  • $0 Monthly Premium
  • Covers both Medical and Prescription Durgs
  • Covers Tier 1 and Tier 2 through the Coverage Gap
  • $3,900 Max Out-of-Pocket in-network and $5,100 Out
  • $240/day hospital co-pay days 1-7
  • $0 co-pay for primary and $45 co-pay for specialist
  • $225 co-pay for outpatient surgery
  • $0 co-pay for labs and up to $150 co-pay for diagnostic procedures and tests
  • Gym membership offered

Advantage Elite

  • $151 Monthly Premium
  • Covers both Medical and Prescription Durgs
  • Covers Tier 1 and Tier 2 through the Coverage Gap
  • $3,400 Max Out-of-Pocket in-network and $5,100 Out
  • $75/day hospital co-pay days 1-5
  • $0 co-pay for primary and $15 co-pay for specialist
  • $0 co-pay for outpatient surgery
  • $0 co-pay for labs, x-rays, diagnostic procedures, and radiology
  • Also offers dental and vision benefits as well as a gym membership

Advantage Enhanced

  • $69 Monthly Premium
  • Covers both Medical and Prescription Durgs
  • Covers Tier 1 and Tier 2 through the Coverage Gap
  • $3,400 Max Out-of-Pocket in-network and $5,100 Out
  • $125/day hospital co-pay days 1-5
  • $5 co-pay for primary and $20 co-pay for specialist
  • $150 co-pay for outpatient surgery
  • $0 co-pay for labs and diagnostic procedures,
