The 2018 Initial Coverage Limit is $3,310. Once you reach the ICL you must pay the next $3752.50 in drug costs. This is the Donut Hole. During this period ALL formulary generics will have a minimum 42% discount and ALL brand drugs will have a minimum 55% discount.
